Get a Sick/ Fit note for work

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence from your GP that you’ve been ill. Instead, they can ask you to confirm your illness by filling in a a Self-Certification (SC2) form which is available from your employer or on the HMRC website. We call this self-certification.

The NHS provides sickness certification only after seven continuous days of illness.

Please complete an admin request form to send your request to the GP. If you have not seen your GP regarding your illness before, an appointment may be necessary to discuss your request.

Going back to work

You should go back to work as soon as you feel able to.

You do not need to see your GP again to go back to work. Your employer may ask for confirmation that you’re fit for work, but it is not standard practice for a GP to provide this.
